A completed, notarized Annual Medical Waiver Form is required to be onsite for every teen in attendance of a Florida District NYI event. These forms are necessary should a teen participating be in need of emergency medical attention. These forms are valid for all Florida events but additional waivers may be required by the venue of the event also. Once a teen attends their first District event their Annual Medical Waiver Form will be kept by the Registration Coordinator then utilized onsite for any event the teen registers for during the year. Each January the Annual Medical Waiver Forms will be shredded and this list will be purged. If the emergency information changes during the calendar year a new Annual Medical Waiver Form will need to be completed and submitted during their next event.
